Monthly Self-Exam Reminder
We collect your email address in order to send you reminders via email every month. You can stop receiving these reminders by simply filling out the form with your email address again, and the Web site will cease to send you emails. Again, we do not use this information to send unsolicited emails to you, nor do we share this information with anyone.

Set Up an Appointment
We collect and review your contact information in order to set up an appointment at the requested time. This information is used to communicate with you during the period of scheduling the appointment. Once the appointment is set up, communication with you due to this information ends (monthly and annual reminders continue as your preferences dictate). This information is never shared with anyone outside of our staff.

Breast Exam Journal
None of the information sent in this online application is captured or stored online. Once you close the browser, all personal information is lost. This application is for you to print and take with you to your doctor or to store in your records. Since no information is stored or captured, there is no possibility that it can be shared or viewed by anyone you do not choose to share it with.

Contact Form
The information given in this form is emailed to our associates who will answer your questions via the appropriate method. Once your question or comment has been dealt with, your information will be purged naturally with the progression of emails through our system.
